@Joannie - I did fill in the provided form in a very pro-vaccination (positive) way, thinking "this is a government survey, but they will probably respond better to a vaccine-positive review' (I am actually of that view, but don't want to start that debate on here . . . ) and the program bombed out right at the end.
I don't have the time now to check this out, (have been interrupted again and again, from doing my office work) - but will follow this up.
I have a feeling it is not a genuine survey; and it is not from a Government source - it should be checked out as to the real source of the 'survey'.
Stranger things have happened, but this will take a bit of time - once I've got my desk clear, I'll give it another try.
