The company can choose to keep bullion in reserve, for example DOM will always announce in their qtr end report cash & bullion held at the end of the period.
I've noticed that someone has been nicely accumulating HEG in the low to mid 30c range, almost daily an order or two of over 100K will appear and then get filled, interesting.
