Form W-8BEN, page-12

A guide to completing the W-8BEN form for individuals, this is how I completed it & it was successful.

You need to complete this form & send it to the registrar by post in order to reduce the witholdings tax from 30% to 15%.
The 15% witholdings tax (or 30% if you don't complete the form in time) can, depending on your circumstances, be reclaimed through your annual tax return (seek advice from your tax expert).

Attached is the blank form I received from the registrar as the one I received via post had a page missing, note that this is a generic form not GQG specific.GQG_W8BEN1_220323_Interactive.pdf

Complete as follows filling in the following as they appear on the share registrar:
Full name of registered holding
Registered address
SRN number (excluding any letters)
Part I
1 Enter Full name
2 Enter Country of citizenship
3 Enter Permanent Address
6b Check this box (with tick)
8 Enter Date of Birth
Part II
9 Put 'Australia' in the box
Part III
Tick box that you have the capacity to sign the form
Sign the form
Enter 'Individual' as capacity to sign
Enter the date you signed the form
Print your full name as signer

LEAVE ALL OTHER BOXES BLANK & POST THE FORM TO LINK MARKET SERVICES.

I'd also suggest scanning or taking photos of the pages before posting.
I didn't get a confirmation from Link that the form was OK or not so you either have to phone to get that or wait until the next dividend to see what is deducted.
