I don't think it was shortcuts here at all. Probably operator error on the machine (yes it happens and has happened before). Leaning over the controls of a EWP and getting stuck between it and a object. Without knowing the facts obviously cant say that for sure but that's what it sounds like. Some work places ensure that a second ticketed operator is on the ground to operate the ground controls in case the bucket operator gets pinned or in trouble. Have seen this type of accident before. The company should ensure it has competent Job Safety Analysis process to identify these sort of risks. Sadly a fella has died, very sad indeed.
