Fortescue told to look into safety

Fortescue Metals Group Ltd has been ordered to get an independent review of safety systems at its three mining operations.

Western Australia's state mining engineer Simon Ridge has told Fortescue to commission an independent engineering review and audit following three major incidents in the past month.

"While the recent incidents have not resulted in any deaths or injury, it is extremely important to clearly identify all factors involved to ensure that there is no similar occurrences that could result in adverse consequences," Mr Ridge said in a statement.

He said one of the department's Resources Safety Mines inspectors was conducting an investigation into an incident on Saturday in which a light vehicle was crushed when a truck drove over it.

The company shut down operations following the incident and the department later issued an improvement notice requiring a thorough investigation of the incident.

Fortescue has until the end of March to report back to the department.
