I believe the easiest way to claim your franking (imputation) credits back is to lodge:

Application for refund of franking credits for individuals.

2 pages to fill in. (really one page).

You can download and print the form from www.ato.gov.au

Alternatively, you can phone and ask for the form NAT 4098-6.2010
