capex is included in an npv calculation in the first year or two where you may have negative amounts..... the discount rate covers off on interest on loans, sustaining capital, run down in reserve/respurces and the decrease in value of a dollar in the future compared to a dollar today. However, with gold this product might actually become more valuable in the future.
NPV is the calculation that is completed when you get a valuation on a mining project. It is what the bod will look at if the company is getting taken over. So NPV is a good indication of what the market capitalisation should trend towards.
