This issue tends to not happen for Phase 1 trials. You do get technical changes from ethics committee to ethics committee, but it is very rare that they will change a Phase 1 trial so much that you can't run it in at a site due to ethics issues. If this was a serious concern we would not start in Australia first, but wait until all sites ethics committees had agreed.
Issues of major differences between sites can crop up in later stage trials, but these are more caused by regulators demanding different things than ethics committee differences.
