    Daniel, calming the horses isn’t executing. Executing is doing countless premortems, where you anticipate what might go wrong and nipping those errors in the bud , and building in contingency plans to deal with unexpected bad luck…

    i wish you had applied for ethics approval in more than one site initially….RAC put all their ethics approval eggs in that one basket…and you got caught out by “bad luck”… I bet next time you run a trial involving multiple sites, you will apply for ethics application at more than one site…the wisdom of hindsight? Learning on the job?

    I think you said at the time that RAC didn’t have the resources to apply at more than one site…well, in hindsight, perhaps RAC needed to invest in more of those “getting the damn trial started” resources. This is what I mean about doing premortems. …
