There is no 'industry standard procedure' for something as non-standard as bisantrene. A good-faith attempt was made, a sort of averaged consensus based on existing info, general past practice and KOL opinion: basically an exercise in extrapolation. This was never likely to work while each person ticked their own box but nobody had an integrated picture. The one person capable of that was of course (pun intended) out of the picture at the time.
