hi mate ive basically forgotten exactly what was necessary i know you had to complete a transfer from the totally inept computershare and a tax form W8Ben ..my best advice is contact westpac global affairs they will supply you with necessary forms and they were extremely helpful not only with setting up but also with operating which can require a few steps..im sorry i couldn’t be more helpful but it was a while ago now and computershare was the guilty party in making things not only late but difficult !..good luck !
