general offer application form

I have received the prospectus for the issue at $0.005.

There is a non-renounceable issue and a general offer and mention of a shortfall application.

I keeping looking at the prospectus and cannot find any forms at all.

Did anyone else get a form or two?

Perhaps the fact is that company doesn't want anyone to apply?

Surely this is illegal.....issuing a prospectus with no means to apply for shares?


50,000,000 share options to be given to the (partial) underwriter too.

There is a short word for sustenance and this is how it look to me. Unpretty sus!
