I know this doesn't perfectly fit your criteria, EVN only hedge 20-25% of their production (recently upgraded guidance 770,000 to 820,000 for 2016), hedging for the next 2 or so years at around AUD$1560 with AISC of around AUD$1000.
They have 7 operating mines, large resources, and still explore... Recently acquired PXG. They have debt, but accelerated payments means it will be paid early, before the end of this year, possibly by Sept.
