I feel like I'm missing something obvious here...
In the Entitlement Issue Prospectus it states, 'Should you wish to accept all of your Entitlement, then applications for Shares under this Prospectus must be made on the Entitlement and Acceptance Form which accompanies this Prospectus or by completing a BPAY® payment, in accordance with the instructions referred to in this Prospectus and on the Entitlement and Acceptance Form'.

This form is a little elusive. Can anyone point me in the right direction?

BTW LT GXY holder, LPD holder as of yesterday!
