SS, Here's my take, using the page sequence of the online form.

1st page: "Enter your details"
These are details of your existing shareholding account

2nd page "Confirm your details"
Confirm that your holding details, as shown, are correct

3rd page "Download scheme booklet"

4th page "Election form"
1. DTC Participant firm name - the name of your nominated DTC participant form
2. DTC Participant 4 digit number - the 4 digit DTC number of the above firm

3. Holder Account Number – broker/DTC participant (optional) - YOUR account number for the account that you have established with your nominated DTC participant firm
4. Holder Account Name - DTC participant/broker - YOUR account name for the account that you have established with your nominated DTC participant firm
5. DTC Participant Contact Name - the name of the person you deal with at your nominated DTC participant firm
6. DTC Participant Telephone Number - the phone number of that contact person, or general phone number for the firm
7. Email address of DTC participant/broker - the email address of that contact person, or general email address of the firm
