I can only speculate on the trigger for the long trend down from '93, but I know something about 2000 when there was a coinciding trade difficulties from Asia and China routes for heroin and internecine drug wars became much more widespread in northern South Americas and through to southern US.
there was a global supply crisis that took some years to settle. this was when Australian reports of chemist raids and the fielding of patients in GP superclinics, specialising in 5 minute consults, to obtain prescriptions of opiates such as fentanyl and Ketamine - both normally used only in operating theatres. it was around 2005 we saw transdermal patches appear in general medical, cancer and palliative wards. it seems the transdermal patches made extraction of the drugs much easier.
