If I was the poor families of these victims I would be talking to a lawyer in the near future in regards to bringing a case of Gross negligence etc etc against the Australian govenment and legal system.
Just because you are a govenment department etc does not give u the right to continually get things wrong and endanger the public and get away with it unscathed.
