How expensive is the injection? How many people will be able to afford to get it every 3 months for a chance at a bit of relief that is transient?

You are the Doc. How much does it cost for back surgery, please include all costs ? I think you will find that our therapy will be very cost effective. 3 months for a transient effect, you pulled that out of your rear end, more like durable and substantial relief for at least 3 years

How many governments will agree to fund it, when it hasn't been compared to steroid injections, which are significantly cheaper?

Take a look at the trial design Doc. Patients have failed standard of care.

Also, investing in a company before any approvals, vs one after a single approval is a huge difference in risk.

Unlike many we have multiple indications .

If the approvals are a given, of course, get in early.

But that's not how any of this works.
