Hi - You need to contact Link Market Services for a form. They now look after PEN's registry of shares. Their number is 1300 554 474. Follow the prompts or speak to a customer service officer and ask for a form to convert PEN options. They will confirm your details (HIN Number and Addy) and then mail you a form.
