    havent had a recent read through the thread to see if all questions were answered, though I can tell you this.

    I called (02)9290 9600 (share registery) today and advised them that I had not received my forms yet. The guy I spoke to straight away emailed me the form which allowed me to bpay them straight away for my entitlement.
