I did have a trip to the USA in early 2007 to visit my rich kid expat daughter and arrived back in OZ with the green stub still in my passport. Didn't bother me much ( good place to visit, been there-done that-don't want to live there)until she tells me that I now might have to come over , or at least transit, for a wedding.
A bit of Googling for I-94W brings up a page about sending lots of supporting info to an address in London , KY.
A trip to my local Dept. of Immigration gave me the form 1359 that I've posted. Hopefully I won't disappear into "America's Worst Prisons" next time I arrive there.
