I don't see how they would factor in when to count a heart attack toward the rate and when not. You can't take one instance of an attack and say "well that was probably one of the %60 who wouldn't have survived so we won't count that one as it will make the results look like an endpoint fail". Any extra survivors who have then have attack are raising both the rate and total of heart attacks aren't they?
