Thanks for that S1. That would explain why a previous IPO application I made was registered as Issuer Sponsored when I was certain I had filled in the form correctly. Silly me, forgot the "Mr".
BTW, I was able to fix the problem online (ETrade) very easily and quickly (was tranferred to Chess within about an hour).
