this is the actual requirements plus 1% fee from haywood securities and your normal fee from shaws

For these stocks to be transferred into electronic form we will need the below from your client to intiate the process with our Custodian, Haywood Securities.

1) Please send in the original Certificate with the back of Certificate signed by the client to Shaws Stockbroking.

2) And please send in the below attached letter of authority with it signed also to Shaws Stockbroking.

Please also be aware of the 110 AUD Courier Fee that will be charged to your equity account when the documents are sent to the Custodian Haywoods for processing.
