We may see more unfold this week from the inquiry. Particularly in regard to a tardy attitude to employee training culture that contributed to this tragedy. This is a reflection of company directors not having knowledge of/or controlling their best asset.....staff.
IMHA when the inquiry anouncement states the obvious that AAD contributed, was negligent in its staff training and responsible - then it will most likely lead to a hefty financial penalty and civil action.
