Well in this case the answer imo is (a) because to get to the high (or should I say medium) grade layers (assuming they are open pitting) they need to go through the low grade rubbish.
Result - big increase in pre stripping capex and ongoing mining costs.
Having said that I have not read a TON announcement for several months, probably almost a year, so I may have no idea what I'm talking about.
