Go to the latest published financial statements for IOP, and you will see that IOP doesn't own any listed investments in the ordinary share capital of any entity.
However, go to the latest published financial statements for IGS, and you will see that the balance sheets of IGS includes listed investments in the ordinary share capital of various entities (which, from the valuation, looks like it is mainly the 4 million shares in Nkwe).
Therefore, Nkwe's top shareholder correctly shows International Goldfields (i.e. IGS), and not IOP, as the owner of the 4 million shares.
