I had similar issues, I applied for shares in a name that did not match exactly with my SelfWealth account name (my fault).
To fix:
1 compare the name on any of your ABB communications from Link with the name of your SelfWealth account
2 download the name change form from Link website and change the name to match your SelfWealth accoun and email to Link
3 When Link have made the name change, print the holding details page showing the correct name.
4 From the SelfWealth website print the Issuer Holdings Transfer Request form complete and send with a photo copy of your photo ID ( driver's licence both sides) and the name change form from step 2 to SelfWealth, (wait patiently).
I was able to do all this by email with Link and SelfWealth without JP certification.
