Answer to question 1: Very often yes. Conventional missile warheads are inclined to explode when hit.
Answer to question 2: Nuclear weapons, and especially those in large missiles, are designed to require a "proper" re-entry to explode. They are very most likely to land unexploded, and usually mostly intact (nuclear warheads are a variety of metals, usually heavy and tough).
