ok so you have to tell him.
a.if there are any treatment options he has to know and be given at least a chance, waiting won't help.
b. if there's not then just maybe he will get some solace out of knowing that there is a reason he's lost such control, put things in order, make some peace, find god maybe or curse the lack of one even. Who knows but not telling is doing no favour imo.
