I think we are all rowing in similar directions to a point, not having read & analysed the '2000 patient' study (ie the protocol, inclusions, exclusions and analyses, and conclusions drawn) I can't add comment, but can't help but wonder.... Has it been published in the medical literature somewhere (perhaps a peer-reviewed radiology journal) ?
The other thing is that when claiming 'sensitivity' (ie true negativity) ; the pre-test probability of an under 40 y.o having breast ca is fairly low on a population basis. I reckon I can find 2000 women under 40 who don't have breast cancer pretty promptly (I apologise if this sounds contrived - none meant)
As far as U/S techs being concerned for their jobs, there is PLENTY for them to do @ present, takes me ten days to get a non emmergent U/S, and thats in a massive tertiary referral centre - no tech is in any danger of losing their jobs yet, even with the entire eastern sea board jumping onto FER!
