A fundamental point here. It's both, actually. But the result has the greatest effect of the two.

An example:
If I wanted George Bush dead, he would not be affected by it.

If I killed George Bush accidentally, the situation would be rather more severe I'm sure you'd agree.

And if I wanted to and in fact did kill George Bush, then that would be worse than the two situations above.
