People from the far right and the "democratic regime change" right often refer to Iranian leaders as "Mad mullahs" . But you never hear them call Israel's leaders "psycho zionists" or "mad zionists" or "zionist crazies" etc etc . But here they are not even pretending anymore that they don't support ISIS in Syria.
