The following is for Queensland. As a percentage,0.05%, the death rate is low but nobody wants themselves or loved ones to be an iota of that percentage when mistakes are easily avoided.
The leaving behind of instruments inside patients is concerning. It's not exactly like tinkering under the bonnet and leaving a spanner on the air cleaner only for it to fall off onto the road!
I respect the medical profession has a difficult task. Negligence occurs in every job. It's unfortunate that death is collateral damage.
