It's a small price for you but a burden for poor people. It is designed to stop people going to the doctor. Sometimes it would stop an unnecessary visit but sometimes it will cause a minor ailment to progress to a more serious one. Hockey doesn't know which is which and neither do you. Plenty of bulk billing doctors are against this doctor tax and they are the people best able to judge.
You can already choose to go to a doctor who charges a patient fee if you can afford it.
