    The drilling mud wasn't heavy enough to hold the pressure.

    The oil/gas mixture blew the mud back up the casing so fast that the blow out preventers had to grab the drill string real tight.

    Well, of course it was oil/gas.

    If it was water we wouldn't go to all the expense of sidetracking back in now, would we?
