I have successfully, and just in time before the deadline, managed to get my Kazia shares transferred from my Superfund to my own name and had them converted /transferred to BNY Melon on the 10:1 basis that was offered to us.
I have received a W-8BEN certificate as part of my documentation and I completed it with my HIN that was provided to me to by Computershare and then mailed the completed W-8BEN form using a provided addressed envelope to Computershare.
Fingers crossed that is all that I needed to do (for a 3 year period) that will take care of withholding US taxes in we get taken out (hopefully)
