Living in one of the first towns in Australia to get NBN, and having spoken to the guy doing the job I will give you a couple of examples why the cost is so high
1/ They had to directional bore the conduit down our road even though Telstra had 3 conduits running up the road and only 1 of them in use.
2/ They cant locate fibre optic so they can't put pits too far apart because they have to be able to run something up the conduit that they can find.
3/ They can only bore about 250m in good soft earth with the plant.
4/ Fibre to the node was always going to be the best option for normal households as the technician that came to our house said we would never use the capability of the copper into our house [ fibre at boundary ] let alone the fibre.

The one thing that gripes me the most is after having built a new home it took 6 weeks to get connected, and after contacting the ombudsman was told that you can't make a complaint about NBN CO only your ISP. Not fair on the ISP if you have no cable for the internet. Super fast net but its pretty expensive if you want full speed with plenty data, I pay about $95 p/m 300 gig 100down 40 up.
