It may sound like a short answer but the best way is to contact the company. I find calling them is the easiest. They are very helpful and will make it a very easy process (you are calling them to give them cash after all). They will send out the required forms with all relevant info. You can fill them out and post back or i believe it can now all be complete online via your broker but i am not familiar with that process. Perhaps someone can explain the online.
