Rebel, Biota's Relenza and Inavir (or LANI - long acting neuraminidase inhibitor - registered in Japan at this stage but subject of the US$235m Barda grant to Biota in the US) should be effective in treating any strain of influenza. research indicates the earlier after onset of clinical signs the better. Resistance to Relenza is extremely rare in contrast there are numerous reports of Tamiflu resistance influenza isolates.
Relenza ((and presumably Inavir) can also be taken as a prevenative in individuals exposed to a high risk situation. Both are anti-viral treatments not vaccines.
They act by preventing the influenza virus from breaking out of infected cells in the rspiratory sysetm by blocking specific receptor sites inside the cell wall.
The virus after multiplying within a cell needs to rupture the cell to liberate large numbers of virus particles and infect other cells. The most severe damage occurs with starins that primarily attack the lungs. This seems to be the situation in the Maryland cluster. Relenza and Inavir are very effective in prevent this spread within the infected individual.
Both Relenza and Inavir are taken by inhalation which puts them right where the action is. tamiflu is taken orally - has to be absorbed through the intestinal tract.
There are also trials in progress around the world on the use of Relenza intravenously on crtically ill patients (ie in intensive care). There are individual reports of miraculous recoveries.
Relenza and Inavir have the capacity to save large numbers of lives in the event of an influenza pandemic. They also have a big role to play in minimising the impact of seasonal influenza - by shortening the course of clinical disease as well as reducing hospitalisations.
