Just because hervaxx played nice with chemo, does not mean it will play just as nice with a MAB combo. Just as any increase in efficacy can be attributed to the combination, any increase in toxicity will also be attributed to the combination.
I would agree (and hope) that added toxicity would not be expected, only added efficay. That expectation however is only due to what we have seen so far in the smallest possible sample of potential combinations. While I am extremely confident in the science so far (to the limit of my understanding), I also understand that there is still a relevant risk that when combining new drugs together there are often unforeseen side effects.
The first alcoholic to take aspirin would have learned that the hard way.
