Yes there is a standard form.
It's a PDF
MEI actually emailed me the form automatically (or perhaps it was the registry)
Once filled out and signed you can:
1. Snail mail it (signed)
2. Email & attach (to MEI)
3. Fax (who has one is blessed)
and.... you need to pay the appropriate amount and ensure that the recipient will need your reference details.
Conversion is 2.4c for a stock that's smashing it!
