If MOD is running these options like they are doing for their listed options, you will need to contact the share registry to get this form. The share registry will want you to prove you are really an option holder (HIN number etc) then they will give you the super secret Option Exercise Form.

These are American style options. They can be exercised at anytime (obviously only logical to do if they are in-the-money)
