It was from Boardroom share registry - there was a cover letter from QMC recommending acceptance of the offer from Moly, and there was a A4 bidders statement and an acceptance form with my own SRN/HIN and a return envelope to post back. I had a look to see if there was a way of accepting online but it looks like it has to be done by mail.
They must have accessed the QMN share registry to get my details so I would probably recommend contacting Boardroom Share Registry - maybe try 1300 737 760 (in Australia) and see if they can help you.
