yes. It’s what comes with spending time at a keyboard and not in the real world. Right now there is no way that humans can be completely eliminated from events. Everything depends on human intervention in someway. And that includes AI which still has human input
I could think of an engineering solution to the road problem - barriers so that humans can’t get on to roads except when cars are stopped but even that’s not fail safe
all the what ifs and maybe solutions have the potential to fail.

good safety systems are based on understanding the multiple possible sources of failure and addressing each of them so there is no reliance on a single aspect
