Then... he (the shaman) became a King and the king appointed a hierarch ("hier"=holy in Greek, "arch"=first, top, etc, also in Greek). Then there was -in some countries it still isn't- the separation and so we have the Archbishops, Muftis, Arch Rabbis and all the rest. Bishop Spong puts it beautifully, if you get the time and the opportunity to buy his books. There are several of them. I foun his "Here I stand" and "Why Christianity must Change" great reading.
