i recall an interview with woodcock FDA saying teh way trials are done should change, to much time money in the present the big BUT was change takes time... suggesting the current don't have the will to change.
if real world examples show 80% of people will die if they have a certain condition there is not point to have blind trial.. as it will condemn those to about 75 % of death.
when a 'pill' is taken in 100 people(80% will die in the same year) ... and 70% survive without placebo it is very very clear it works.
