There is a form of Medicare there but it's not universal it's means-tested and this helps support a form of insurance.
There are also some government funded hospitals.
Hospitals often provide charity care which is uncompensated by the patient ,but funded by government subsidies and other funding sources.
The hospitals have purchasing committees. It's complex and a lot of paperwork is involved to deliver a new service.
