Get whatyou’re saying but can’t help seeing things a little different. For me ArTiMistworks better because it didn’t work as it should. From TGA report need tocorrect myself - ArTiMist 90% parasite clearance time at 15h looks similar to whatartesunate suppository (RAS) achieves. Both inferior to IV artesunate (10.2h)[1].
It was proposed that there is more SL absorption in healthy adults than sick youngchildren due to longer SL retention time. In addition, in children, someartemether is swallowed and absorbed via the gastrointestinal (GI) tract (TGAReport).
There wasevidence of FP metabolism in both absorption phases in children with malaria. Comparedto a previous study, in Melanesian children 5 to 10 years old withuncomplicated malaria who were treated with conventionalartemether-lumefantrine tablets, there was no significant difference in thedose 1, dose 6, and total AUC0–∞ of artemether or DHA or in the totalDHA-to-artemether ratios [2].
Sublingual/oral dosinghas the added benefit that the swallowed fraction may undergo FP metabolism ofartemether to DHA, with higher relative amounts of the more potent metabolitebeing present.
Walter WalterReed Army Institute of research (WRAIR) (explicit abbreviation) has chosenartesunate (AS) injection as the blood stage compound for severe andcomplicated malaria and it has been demonstrated that AS is a pro-drug of DHA[1], which has same efficacy to AS and is three to five times more active thanthe other artemisinin derivatives. Artesunate is a more potent anti-malarialdrug than arteether and artemether, which may be explained by the conversion toDHA with the DHA/AS ratios of 0.31–2.74 in animal species, and 2.81–9.71 inmalaria humans. A study demonstrated that only DHA was rapidly effectiveagainst all stages of the parasite life cycle and completely inhibited theparasite growth within the shortest exposure time when compared to all otherartemisinin drugs. It is also felt by many clinicians that AS administeredintravenously is the most effective for treatment of severe malaria due to thehigh DHA levels [3].
