This is not new.. its been happening year after year.. and I dont think anybody can prevent this.. BHP has world class incident management system.. they used to report when I used to work there for not holding the railings of the stairs.. and used to send KB articles to refresh the safety course.. where you have heavy machines and human involved you will have accidents..
